1. TELEHEALTH
For your safety, given COVID-19, new patient visits may be via telehealth with on-site exams as needed.
2. OFFICE HOURS
Monday-Thursday 8 AM-3PM
3. COMMUNICATION
All new patients receive one free 15-minute phone consult. All future communication is via telehealth or office visits. The Patient Portal is used for scheduling and billing issues. For your safety, email and text is not used for medical consultations.
4. AVAILABILITY
Phone calls, emails, and messages on the Patient Portal are returned within 24 business hours (excluding weekends, holidays). If away for more than 24 business hours, patients will be notified.
5. CANCELLATIONS/MISSED APPOINTMENTS
No show or cancellation less than 24-hour notice will incur $25 fee.
6. PRESCRIPTION REFILLS
For your safety, refills are provided during appointments only.
7. LAB RESULTS
Lab results are reviewed at office and virtual visits and not via email.
8. PAYMENT
Copayments are to be paid at the time of service. If your insurance is not a plan we participate in, payment in full is expected at each visit.
